Social Care, Health and Education are working together to co-produce a new care and financial model, working with people and their families with disabilities and other voluntary organisations.

The project is for people who have lots of different needs, sometimes called complex needs. This is where people have lots of different needs like learning disabilities, autism, physical disabilities, or considerable needs to do with health and education.

The aim of this work is to focus on providing people with the right support at the right time and enabling people to be able to take control of their support.

In Hampshire we have started the project by working with young people who are in transition, who are between 14 and 25 years old and have complex needs.
