Young Epilepsy Transition Support is a free project working with 16-25 year olds, with epilepsy living in Hampshire. Whilst young people with epilepsy have support from their health professionals, we recognise that other areas of their lives may be impacted quite severely, such as housing, independence, developing their personal, social skills and feeling very isolated. We encourage young people to have a sense of purpose, gain a network of friends and to live a well-rounded life. The Transition Support Project works with young people to support them to identify and overcome issues or barriers to further education, training, employment or any other areas of their lives. The project also works alongside other organisations, to support them by supporting your young people such as at appointments, activities, volunteering and employment.
