What does your service do?
The HHFT Community Paediatric Medical Team consists of a team of paediatricians who identify, assess and support children and young people with a wide range of developmental disorders and disabilities. In partnership with children, young people and their families we work in a multidisciplinary team, liaising with GPs. Health Visitors, School Nurses, Therapists, Mental Health Services (CAMHS) and Hospital Doctors, as well as with Education and Social Care. Our aim is to identify your child’s health needs and areas of difficulty. Sometimes this will lead to us making a specific diagnosis. The first time that we meet your child you will have a longer appointment to allow a thorough assessment. This includes listening to your concerns and gaining relevant information. It is important we don’t miss any key points therefore you may be required to repeat information that you have previously told other professionals. It would be helpful if you could bring with you copies of any reports from other relevant professionals (teachers, therapists etc.) Your child will usually have a physical examination and often a developmental assessment will be performed. We will provide you with initial feedback but often it is necessary to perform further tests or assessments before a diagnosis can be given. Once our assessments are complete we will ensure that your child is signposted to the relevant support and specialist services. Your child may be discharged from our service at this point but we sometimes provide medical follow up in clinic if there are chronic and complex health needs.
Who does your service provide for?
We provide services for children from birth up to their 16th, or in some cases 19th, birthday. Your child may be referred to the service if they are showing signs of developmental delay in one or more areas of their development or have an identified condition such as: *Physical disability - e.g. cerebral palsy, muscular dystrophy, spina bifida *Learning difficulties (mild, moderate, severe and complex) *Neurodevelopmental disorders including Autism Spectrum Disorder (assessment and review for preschool referrals; school age referrals may be seen and reviewed depending on physical, medical or other non behavioural needs), Developmental Coordination Disorder, language and communication disorders. *Impairments of special senses – hearing and vision impairment *Acquired disorders e.g. brain injury due to trauma or illness, extreme prematurity *Complex disorders e.g. genetic syndromes If your child is undergoing assessment for special educational needs then we will be requested to provide a medical assessment and summary of relevant medical information. We also carry out medical assessments for Looked After Children and those going through the adoption process.
How can I start using the service?
OurThere is no direct access or self referral to our services. Referral to our services must be via another professional. This professional can be the Health Visitor, GP, Special Educational Needs Coordinator (SENCO), Inclusion Officer (INCO), School Nurse, Acute Paediatrician, Therapist, Social Worker etc. Referral can be by letter or by means of a specific referral form. All children referred to our services will be seen within 18 weeks (as per national guidance). Community Paediatric Medical Services are provided on the NHS so there is no charge. You can not add our services using your personal budget.
Where is it located and what areas does it cover?
Our clinics are located in the Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital (Winchester), Chase Hospital (Bordon) and Andover War Memorial Hospital. Some preschool clinics are run at local Children’s Centres. If your child attends a special school and has medical needs they may be seen in their school by a Paediatrician. Each clinic is located in a distinct geographical area (patch) and has a lead Paediatrician. How are decisions made about who can use your service?
All referrals to our service are looked at by a paediatrician. If your child has been appropriately referred to our service you will be offered an appointment within 18 weeks. If further information is required for a decision to be made then we will contact the referring professional for this. Very occasionally we may decide that another service is more appropriate to meet your child’s needs. In this case we will advise the referrer of our decision.
How do you communicate with service users and how are they involved in decision making?
Your first appointment will always be a face to face and two-way discussion. Both your and your child’s concerns, opinions and beliefs will be sought as part of the consultation. If your first language is not English then you must inform us of this before the appointment so that an interpreter can be present. If you or your child uses a specific communication system please let us know so that we can liaise with the Speech and Language Therapy service to provide support during the appointment. A copy of the assessment report will be sent to you. We may ask for your permission to share the assessment report with the other relevant professionals who work with your child. We have a wide variety of leaflets and links that we will provide you with after the consultation if this is relevant to your child’s needs. You may be asked to contribute to our service feedback surveys during your visit to us so that we can continue to improve our services.
Is your service fully accessible?
Our clinics are held in buildings that are DDA compliant and are fully accessible. In addition we have hoisting facilities, toileting and changing facilities if required. Disabled Badge Holder parking spaces are available on all hospital sites.
What training are the staff supporting children and young people with SEN and Disabilities had or are having?
All the staff have specialised in assessing and treating children with special educational needs and disability. They have specialist training qualifications and undertake regular continuous professional development training in these areas.
How will I know my child is safe?
All staff are trained in Safeguarding Children and are DBS checked as a routine part of their job. Monitoring of competence in this area is mandatory. Our overall services are monitored by the Care Quality Commission (CQC).
Who can I contact for further information?
You may have a trusted professional already involved with your child who you can discuss your concerns with. As our service takes referrals from a wide range of professionals they will be able to advise you whether a referral to our services is appropriate.
